With the right knowledge and the right treatment options, you can accelerate your recovery without the fear of re-injury or setbacks (which unfortunately, happens to many people - we will explain why further down the page).
The 3 Stages of Soft Tissue Healing for the Elbow
Regardless of what soft tissue condition in the elbow you may be suffering from, if the pain is signficant and/or impacts your day to day life - get to a physician for a proper diagnosis. Once you know what you are dealing with, you and your doctor can now confidently undergo a proper treatment plan. In some cases - such as a complete tendon tear - there is no option other than to undergo surgery. If the diagnosis has confirmed a soft tissue injury such as a strain or partial tear, your doctor will most likely prescribe a conservative treatment protocol including rest, elevation, and the use of an Cold Compress or Ice Pack for reduction of pain and swelling. When dealing with a soft tissue injury (an injury to muscles, tendons, ligaments and/or cartilage), there are three major stages of the healing process. These stages are as follows:
Step 1: Reduce The Initial Inflammation
Inflammation is the body's natural response to an immediate elbow injury and is a normal part of the healing process - helping to reduce tissue infection in the early stages of injury. Swelling, pain, heat sensation, redness, and loss of function are the main symptoms experienced.
The combination of rest, topical pain relief cream and minor amounts of cold is the gold standard in medicine for minimizing tissue damage and reducing inflammation after injury or activity. It serves as a critical bridge into the next phase of the healing process.
Step 2: Enhance Blood Flow to the Injured Elbow & Surrounding Tendons, Muscles & Ligaments
Tendons,ligaments, cartilage, and some muscle fibres are dense tissues - as a result, they naturally receive limited blood flow. Limited blood flow may not seem like an issue when everything is working fine, but this is precisely why injuries to these tissues take so long to heal. When dealing with injured soft tissue - the real conundrum is how to increase blood flow to these tissues to help the body heal the injury.
- Rigorous exercise is out of the question as it will lead to further injury.
- Visits to the clinic are helpful, but that only happens a few times a week.
When you have a muscle or tendon injury, there is reduced blood flow to the injury site - inflammation and reduced movement (lack of activity or on-going immobility) will reduce the flow of blood - and if you think about it, this makes sense. Moving the joint(s) where there is swelling is very painful, so it is very common for elbow tendonitis sufferers to have significantly reduced mobility; over time, this leads to atrophy - a shortening of muscles, tendons and ligaments in the elbow joint. As the elbow slowly heals, you run a high risk of straining your muscles and tendons as they have become shortened due to lack of use - leading to more soft tissue injures and possibly putting you into a chronic soft tissue injury cycle.
It is through the blood the body carries the nutrients, oxygen, and antibodies that injured tissues rely on for recovery.. It is well known that increased blood flow helps your body accelerate the healing process.
Step 3: Recognize That Healing is a Process

What Is Tennis Elbow? ... What Is Golfer's Elbow?
Epicondyle: any of several prominences on the distal part of a long bone serving for the attachment of muscles and ligaments. Those bony bumps on the inside and outside of your elbow are called epicondyles.
Epicondylitis: inflammation or damage to the area of an epicondyle of bone.
Tennis Elbow and Golfers Elbow are two different manifestations of epicondylitis and are very painful. These injuries typically "flare up" over a period of years unless treatment is properly addressed. The most common reason for these injuries is from repetitive motion tasks, and the first thing your physician will tell you is to stop doing whatever it was that caused this problem in the first place. This is a big problem for many patients when you are trying to heal this injury, as it typically means you have to stop working. If this sounds like you, then you know how important it is to heal your elbow injury quickly and make sure it doesn't happen again when you get back to work.
Tennis Elbow is a form of tendinosis that is derived from swelling and tenderness on and around the extensor tendons that attach to the lateral epicondyle (small bony part on the outside of your elbow). Tennis Elbow affects tendons located in your forearm that are used for extension of your wrist and fingers.
Golfers Elbow is also a form of tendinosis that is derived from swelling and tenderness on and around the tendons that attach to the medial epicondyle (small bony part on the inside of your elbow).
The affected tendons are just different than that of tennis elbow. Golfers Elbow affects the tendons in the forearm that are used for flexion of the wrist and fingers (ie. make a fist).
Golfer's Elbow = Tendinosis on the Inside of the Elbow (medial)
Tennis Elbow = Tendinosis on the Outside of the Elbow (lateral)
What is Tendinosis?
Tendinosis is a non-inflammatory degeneration of the collagen fibers of a tendon.
When you are diagnosed with tendinosis, one or more of the extensor tendons in your forearm have become chronically damaged. This means that the speed of tissue damage has exceeded the healing process over a significant period of time. Eventually the accumulation of damage to the tendon is not able to heal properly - the tendon fibers break down to a point where the once straight, strong, flexible bundle of collagen fibers become weakened by abnormally formed fibers that look more like a mess of strands.
Tendinosis tissue breakdown is often caused by repetitive stress on the tendon fibers. This means that tendons suffer from overuse without enough time to rest in between. Lack of adequate rest means the tendon's fibers haven't had enough time to heal properly before they are stressed again.. and as such sustain even more damage. Tendinosis can also occur following an acute injury to the tendon, however, it does not appear until long after the injury occurs. If an acute injury is not given enough rest to heal, re-injury may continue to occur faster than the tendon can heal. This will eventually lead to degeneration of the collagen fibres as well.
Since there is no inflammation with tendinosis you may not experience swelling, heat or redness; however, you will experience pain, tenderness and stiffness within the tendons of the elbow and forearm where the tissue degeneration has occurred. When severe enough, the pain will radiate into the hand and even into the fingers.
Tendon damage and/or tendinosis damage are quite common in the elbow joint as these tendons are subjected to high levels of tension on a frequent basis. The extensor tendons (used when extending wrist and fingers) and flexor tendons for closing wrist and fingers) are used in thousands of tasks, often handling very high levels of stress. Combined, these 2 sets of tendons handle almost every task imaginable (ie. handshake, typing, driving, using a hammer/racquet/tv remote/phone, golfing) that you might do with your hands, forearms or fingers and partly for such reasons, healing can take a long time. Especially without proper treatment.
Continuous re-injury and build-up of scar tissue while staying active means you will have a greater chance of winding up with on-going elbow pain, more tearing in your tendon, tendinosis (thickening of your tendon) or even a full rupture.
If you have pain and inflammation in the tendons of your elbow or muscle tissue, it's very important to heal your injury completely. You must avoid the buildup of scar tissue (and the corresponding mobility limitations that inevitably follow). If you don't, your elbow injury may plague you forever. This is why it is so important to continuously use conservative treatment tools to heal recurring soft tissue damage before it can build into something much bigger.

3 Grades of Epicondylitis (Tendinosis at the Epicondyle)
Grade 1 Epicondylitis
This is the mildest form of the condition, and is characterised by the feeling of a general ache in either the lateral (tennis elbow) or medial (golfer's elbow) part of the elbow, radiating into the forearm. The feeling occurs during the activity and slowly disappears once the activity is stopped. If a correction to the activities is not made, then the condition will progress to a grade 2 level. Correction to the activity could include changing routines (injury due to repetitive movement), the way the activity is performed (mechanics of form/performance) and/or more rest period length between activity periods.
Grade 2 Epicondylitis
A grade 2 epicondylitis condition typically means that the pain becomes more pinpointed and tender at the epicondyle location. Edema (excess fluid build-up) often occurs at this stage, and the symptoms continue to occur even after the activity is stopped.
As a tennis elbow condition progresses, tingling in the thumb and index fingers may occur with radiation of pain along the forarm.
As a golfer's elbow condition progresses, tingling in the two smallest fingers may occur with radiation of pain along the forarm.
Grade 3 Epicondylitis
If you are suffering from a grade 3 epicondylitis condition, the pain, swelling and tissue damage are now causing weakness that is interfering with your day to day activities. Further to this, you will now be involuntarily compensating for this pain by using your shoulder, neck and thorax rather than your elbow when performing activities.
What Having Epicondylitis Really Means
Epicondylitis injuries can and do persist for years unless treatment is properly addressed. They are quite common because the elbow joint is in constant use during an average day. Everyday tasks such as holding a cellphone, using a keyboard, and leisure activities can contribute to a gradual weakness in the elbow - this makes the muscles and soft tissue within the elbow joint prone to straining, tearing and degeneration over time. One of the most commons ways to contract epicondylitis is through overtraining - as it is easy to overdo and not really notice a problem until it is already too late.
The biggest challenge one will face with a tennis elbow or golfer's elbow injury is at work. It is highly probable that when at work you will be stressing the elbow is some form or other. In an office setting, sitting for extended period of times and working at a keyboard will stress the forearms, as does repetitive tasks in professions such as dentistry, cooking and others that require gripping or constant hand & arm motion. For tradespeople, warehouse workers, and those in a factory setting, there is constant lifting and moving - both of which greatly stress the elbow joint.
This is why it is important to heal your elbow tendinosis injury quickly and effectively - so you can start living a normal life again; both at home and at work.
Elbow injury sufferers will always be at risk of re-injuring their elbow. It's just not possible to stop everything and rest the injury properly. Everyone has demands that make them keep going and when we are active we prevent the elbow from healing completely.
An Elbow Tendinosis Injury Does Not Have to Persist For Months Or Longer. Here is Why It Does For Many
Recovery from an elbow tendinosis injury can take a long time, especially when some of the less effective, traditional methods are utilized. This is simply because many treatments either focus on masking of pain or are too inconsistent to provide sustainable improvement.
1: Tennis Elbow / Golfer's Elbow Sufferers Continue to Aggravate & Re-Injure Their Elbow
It goes without saying that anyone suffering from a soft tissue injury in the elbow should want to minimize their chance of worsening the injury, or re-injuring it once it has started to get better. Obviously, a worsening injury will delay the healing process, but what's worse is that every re-injury and additional healing cycle increases the amount of time that your joint is immobilized. Time is no friend to an immobilized joint - you lose strength in the joint but worse than that, you lose the range of motion in your joint via atrophy (your flexible tendons, muscles and ligaments slowly shrink, decreasing joint elasticity). The more time that goes by with an immobile joint, the more likely you will wind up with a chronic joint problem (long term injury such as tendonitis or persistent bursitis pain). Basically it means that your joint will not perform as well as it once did and it becomes more prone to injury again later on.
If you have a tendon or muscle injury in your elbow, it's very important to heal it quickly and completely. Minimizing the healing time of your injury should be an obvious goal, as a painful elbow injury will typically limit your ability to go about your daily routine. It is generally accepted that tennis elbow will last anywhere from six months to 2 years; if left untreated, pain that started at the elbow will travel down the arm to the wrist. Eventually, simple tasks such as shaking hands or turning a key in a lock can become so painful it becomes impossible.
1: Cortisone Injections Can Cause More Harm Than Good
A cortisone injection can be quite powerful in helping reduce inflammation and/or pain. Keep in mind that it is generally recommended that no more than 3 or 4 injections per year are advised. Further to this, corticosteroids are often not appropriate for people with some metabolic conditions as such an injection can raise blood sugar levels. The issue we find with cortisone injections is that people get a false sense that their injury is healed because they are not experiencing the pain they once did. Some individuals even engage in activities they should not be doing because they "no longer feel pain". Once the effects of the cortisone wear off in a week or a month, people often find their condition persists . This of course, results in a prolonged recovery. This is why we are seeing less cortisone injections into tendons; there are many downsides as outlined here (https://www.ncbi.nlm.nih.gov/pubmed/24074644).
Merely masking the pain is not a solution; it is just a temporary fix. Cortisone does not provide a permanent contribution to any stage of healing.
3: Visits to the PT Clinic Helps (We Always Recommend It), But It Can Be Costly And Inconvenient
While the benefits of PT are very real and we believe it to be integral for recovery from soft tissue injuries, the problem is that people only attend sessions a few times a week.
To truly provide a long term solution to combat chronic tendon damage, you need to be receiving multiple treatments on a daily basis and get rid of the damage asap... and not just any type of treatments, but ones that focus on generation of new healthy tissue - along with proper synthesis of that tissue as well.
Treatments for tendon and muscle injuries are cumulative in nature...meaning the more the treatments are administered, the faster the benefits can be experienced.
Who these days has time to visit a therapist on a daily basis? Not only is your time limited, but the cost of daily PT sessions is out of reach for most people.
Therefore, the focus needs to be on adding treatment options that can be done frequently, from the comfort of your own home (and even the workplace if the opportunity presents itself). We will discuss these options later on down the page.
4: People Tend To Focus Too Much On Cold Treatments
This is the third and final reason for extended soft tissue injury causes. We see it all the time - people for weeks and even months only focus on using cold.
Doing too much cold will actually inhibit the recovery because you are starving the injured tissues of the blood flow they need to repair and rebuild the collagen. Cold restricts the vessels - reducing the blood flow to the tissues that truly need it most.
Cold treatments should be more focused on in the earliest stages of an injury with much less focus in the mid and latter stages.
The truth is, there will always some mild inflammation associated with tissue injuries. However, that inflammation will always remain UNTIL you begin to focus on healing the tissue. Once soft tissue begins to heal, the inflammation will then subside more permanently - not just temporarily like with cold treatments.
As an analogy, think of a home with a leaky roof. You can place a bucket on the floor of the attic whenever it rains - and this will help temporarily, but does it truly address the situation? Obviously the answer is no. Not until you focus on repairing the structural issues with the roof - will the problem be actually solved.
The same concept applies to injured soft tissue - you need to focus on reinforcing growth of flexible, strong tissue. To do this, you need more blood flow reaching them, not less.
Tennis Elbow & Golfer's Elbow Injuries - They Certainly Are Life Changing Aren't They...
If you've been living with the agony of an elbow tendinosis injury for some time now, it's almost a sure thing that you're feeling discouraged, tired, and worn-out. Seemingly simple chores are now impossible - like attaching your seat belt or putting away groceries. The constant elbow and forearm pain has no doubt interrupted your time at the computer, just adding to the long-term misery that this condition can bring on.
A long-term tendon injury to the elbow can cause extreme pain and reduce your ability to move. It can be very challenging to keep a positive attitude and outlook when suffering from the pain and immobility that this condition can bring. As you know it is just as challenging to keep from re-straining the muscles and tendons that make up the elbow - this is really the core issue of tendinosis... re-injury.
If you choose not to deal with your epicondylitis, you may end up in a long term downward spiral. What ends up happening is that the elbow pain will freeze you up over a long period of time, cause overcompensation injuries, and decrease the range of motion in your affected joint (which can by itself cause secondary soft tissue injuries). This will definitely set back your recovery and in some cases, it can trigger a long term problem that never goes away (reduced joint mobility, tendonosis, athritis, numb fingers). Believe me when I say this - it is important to not ignore your elbow tendon injury.
Tennis Elbow Is A Painful, Persistent Injury:
It Can Lead to Other Conditions and Injuries.
Every time we use our injured elbow, damaged and swollen tendons and muscle tissue move; when they are inflamed, every movement hurts. With injured tissue, pain happens - sometimes a LOT of pain, so we try not to move it. So, when doing something we need to use our elbow for (getting dressed, driving, talking on the phone), we start to use the shoulder and even the neck more. If we can, we use our other elbow instead, even though it is inconvenient and usually the non-dominant elbow.
A couple days later (after purposefully avoiding movement of our elbow), the pain has gone down (as well as the swelling) and this is where the trouble begins! The pain disappeared with the swelling but we still aren't fully healed! Not knowing this, we start using our elbow normally again and then the pain shows up again like it did before - all because the injury wasn't fully healed in the first place.
Eventually, we use the other elbow more and more - or even use our shoulder and neck instead of our elbow for a lengthy period of time. This is why these other areas start to hurt - they are being overused now. Soon, aches and pain can become commonplace in the upper body - all as a result of the original elbow injury and the body's instinctive nature to "protect" the orginal elbow injury - all because the injury wasn't fully healed in the first place!
We continually re-injure the elbow through our daily activities and now this injury has become a chronic elbow injury.
Muscle imbalances result, placing tension on bone and softer tissues - leading to a misalignment within the upper body. The extra stress on the opposite side of the body will also result in weakness and fatigue of the tissues - increasing the chances they can eventually be injured or gradually degenerate. This is a textbook example of how you get an overcompensation injury. In some cases, physical therapists warn that overcompensation can be very dangerous as it will affect gait or alignment on a permanent basis.
An injury to one elbow can easily lead to straining in other areas, making an overall recovery hard to initiate. For example, a serious muscle strain in the right elbow can often lead to over-straining of the left elbow. Most commonly, the injury occurs on the dominant side, so the risk of straining the other (weaker) side increases. The longer the injury (and corresponding pain) persists, the greater the chance that you will sustain more strain from overcompensation, eventually leading to issues in those areas as well. To minimize potential secondary injuries, know that it is important to deal with your elbow injury quickly and completely.
Truly, chronic elbow injuries can become a vicious cycle - which is why it is absolutely critical to focus on methods that improve and strengthen the injured tissues in a timely manner. The faster you truly heal, the less chance you have of sinking into a downward spiral of re-injury, scar tissue growth and overcompensation injuries.
Okay, So I Have an Elbow Tendon Injury...What Should I Do Now?
If you unsure about what is causing your elbow pain, the first thing to do is consult your physician; only your physician can give you a proper diagnosis and from this, determine a course of proper treatment. After seeking medical advice, your physician will determine the degree of severity, whether infection is present (important!) and map out a treatment plan for you. In some cases, your physician may recommend surgery, in which case a conservative treatment protocol would be recommended after your skin heals up from surgery.
For non-surgical cases, your physician will almost always recommend conservative treatment options. A general outline of our conservative home treatment recommendations follow..
Stage 1: If the elbow injury is very recent - use a good quality cold pack and Arnica Pain Relief Cream.
Note: If you are suffering from a tendinosis injury that has been going on for awhile, then skip the cold treatment as inflammation will be minimal. Anytime you are experiencing significant pain, you may find our Arnica Infusion Pain Relief Cream to be quite helpful.
If you are experiencing pain and swelling in the very early stages of the injury (first 48 to 72 hours), the first step in a conservative treatment protocol would be to focus on reducing the discomfort by applying a quality cold pack to the injured elbow, then a high quality pain relief cream such as our Arnica Infusion Cream (after the cold pack treatment).
Not only will you use your cold pack for the first 48 to 72 hours after the injury, but you should also use it during other stages of the healing process. Some examples include...
- after any activity that stresses the elbow in a moderate way. It is wise to quickly apply cold to not only reduce the pain, but to also minimize tissue damage that may have occurred during the prior event.
- If you have a job that requires you to place some demands on your injured elbow. If that is the case, use cold immediately following work and if the opportunity allows - use it for 15 to 20 minutes during lunch or another break time.
- If you are in a post-operative situation where cold compression would be utilized heavily for at least the first few weeks following the procedure. Similarly, if you are scheduled for upcoming surgery, then you will need a high quality cold device to use.
Having a Cold Compress or Ice Pack available at home to use if and when the need arises is helpful. Many of you already do so.
However, too many people over-focus on cold while ignoring the most critical aspect to healing - nourishing the injured tissues with a healthy supply of blood flow needed to repair and rebuild the injured tissues.

Stage 2: Focus on Increasing Blood Flow To The Elbow - This is How the Body's Healing Process is Stimulated
Even though the concept is simple, improving blood flow to injured tissues can be difficult. When the injury is elbow related, the challenges are even greater. Traditional methods require your muscle to move to promote blood flow (exercise), but that same motion that promotes blood flow can at times lead to making your pain and condition worse.

Atrophy in the Elbow
Shortened muscles and tendons due to lack of use is a real problem during recovery. When you start to feel better and use the joint, the shortened muscles and tendons get over-strained, introducing small microtears. These microtears start the formation of scar tissue, which unlike muscle or tendon, is completely inflexible.
